This registered large quantity generator does not file a medical waste management plan (MWMP) with San Joaquin County <br />Environmental Health Department (EHD). <br />- A large quantity generator required to register with the EHD shall file with the EHD a MWMP on forms prescribed by the EHD. <br />HSC117960 <br />A current Medical Waste Management Plan was not available. Submit a revised copy of the Medical Waste <br />Management Plan to the EHD by 3/1612020. <br />13. This registered large medical waste generator does not maintain individual treatment records and shipping and tracking <br />documents for 2 years. <br />- A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, and <br />shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. HSC117975(a) <br />The facility is under new management as of June 1, 2019, and has not retained prior shipping and tracking records. <br />Facility personnel stated that waste is pick-up once per week. The pick-up frequency could not be determined from the <br />invoices in the facility. Submit evidence of medical waste shipments since June1, 2019 to the EHD by 3116/2019. <br />23. Biohazard bag does not meet the standard set forth in HSC 117630. <br />- Ensure that the biohazard bag is a red disposable film bag that is impervious to moisture and tear resistant. HSC 117630 <br />The small biohazard waste bags do not. Discard the small biohazard waste bags, and replace with bags that meet both <br />the ASTM D1709 and the ASTM D1922 standards. This is a repeat violation. Submit evidence of correction to the EHD <br />by 3/16/2020. <br />24. Medical waste is not segregated at point of generation. <br />- Medical waste shall be contained separately from other waste at the point of origin. HSC 118275(a)(1) <br />Paper waste and an alcohol pad contaminated with blood were discarded in a sharps container. Submit evidence of an <br />in-service training on the segregation of sharps, biohazard, and solid waste to the EHD by 3/16/20. <br />25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br />labeled. <br />- Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight -fitting cover, and be kept <br />clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or "BIOHAZARD" on the lid and on the <br />sides so as to be visible from any lateral direction. HSC 118280(c) <br />The biohazard waste container in the south storage closet is missing a required label on the front of the container. <br />Submit photographic evidence pf correction to the EHD by 3/1612020. <br />29. Intermediate storage area is not secured or marked with proper signage. <br />- Intermediate storage area shall be either locked or under direct supervision or surveillance and marked with the international <br />Reinspecion on/about: A reinspection fee of $152 per hour may be charged.
