Pharmaceutical waste is not segregated for storage. <br /> - Pharmaceutical waste shall be segregated for storage in accordance with the facility's medical waste management plan. <br /> Shipping container shall be in compliance with DOT and DEA. HSC 118275(a)(6). <br /> (1) Pharmaceutical waste was discarded in a sharps container. Submit evidence of in-service training on the <br /> segregation of pharmaceutical waste for employees handling pharmaceutical waste to the EHD by 2/19/19. <br /> (2)The pharmaceutical waste container is not labeled with the date it was put in service. Submit evidence of correction <br /> to the EHD by 2/19/19. <br /> All corrections are due by 2/19/19. <br /> Notes: <br /> 1) Biological Environmental Services&Waste Solutions is the licensed medical waste hauler for both biohazardous and <br /> pharmaceutical waste. Biohazardous waste is picked-up weekly. Pharmaceutical waste is picked-up in the week the <br /> container is filled which is approximately every 3 months. <br /> 2) The Medical Waste Management Plan and annual training records are available for review. <br /> 3) The biohazardous waste bags meet both the ASTM D1709 and D1922 standards. <br /> 4) The facility generates over 200 lbs. of medical waste at least one month of the year. <br /> Reinspecion on/about: A reinspection fee of$152 per hour may be charged.
