HSC 118275 -Medical, biohazardous,sharps,trace chemo, pathology, or pharmaceutical waste shall be <br /> segregated and contained pursuant to this section. The container shall be properly labeled. <br /> OBSERVATIONS: <br /> 1. The sharps container in the lab/processing room was not closed when it was not in use. <br /> 2. A tongue depressor was observed in the sharps container located in MSA 2 (see PHOTO 2). <br /> CORRECTIVE ACTIONS: <br /> 1. Ensure the sharps container is closed when not in use. Train staff of proper containment of medical waste and provide <br /> training record(s)to the EHD. <br /> 2. Train staff on proper waste segregation. Provide training record(s)to the EHD. <br /> 25. HSC 118280 -Biohazard bags shall be containerized and stored pursuant to this section. <br /> OBSERVATIONS: <br /> 1. The biohazard containers throughout the facility were not properly labeled (see PHOTO 1). <br /> 2. Facility records indicated the biohazardous waste was not picked up every 7 days prior to March 22, 2023. <br /> CORRECTIVE ACTIONS: <br /> 1. Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be <br /> kept clean and in good repair. Container shall be labeled with the words"Biohazardous Waste" or"BIOHAZARD" on the lid and <br /> on the sides so as to be visible from any lateral direction. <br /> 2. A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste above <br /> 32°F onsite for more than 7 days. <br /> 30. HSC 118310 -Designated accumulation area shall be secured and marked with proper signage. <br /> OBSERVATIONS: <br /> The accumulation room did not have the proper signage. <br /> CORRECTIVE ACTIONS: <br /> A designated accumulation area shall be secured by use of locks on entry doors, gates, or receptacle lids and marked with <br /> warning sign, "CAUTION—BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED PERSONS KEEP OUT', in <br /> English and in another language determined to be appropriate by the infectious control staff or LEA. <br /> Correct all violations and provide evidence of corrections to the EHD <br /> by October 25, 2023. <br /> Notes: <br /> 1. The inspection report was emailed to Jackson Felt, Center Manager, at Jackson.felt(c�CSLplasma.com. <br /> 2. Stericycle is the medical waste hauler for the facility. <br /> 3. Gauze/tape attached to needles were observed in the sharps container. This waste stream is approved by the EDH. <br /> 4. Records were reviewed from October 5, 2022 to August 10, 2023. The facility began operating in October 2022. <br /> 5. Ensure tracking and treatment records are on site and available for review for 2 years. <br /> 6. Staff training records are available online. <br /> 7. The Medical Waste Management Plan shall be updated and a copy shall be provided to the EHD. <br /> Reinspection on/about: A reinspection fee of$162 per hour may be charged.
