HSC 117630 - Biohazard bag shall meet the standards set forth in this section. <br /> OBSERVATIONS: <br /> The biohazard bags do not meet the ASTM requirements for transport off site (see PHOTO 3). <br /> CORRECTIVE ACTIONS: <br /> Biohazard bag does not meet the standard set forth in HSC 117630. Ensure that the biohazard bag is a red disposable film bag <br /> that is impervious to moisture and tear resistant. Biohazard bags must meet ASTM D1709 and ASTM D1922 standards for <br /> transport off site. Provide evidence of purchase or photographic evidence of correction to the EHD. <br /> 25. HSC 118280 - Biohazard bags shall be containerized and stored pursuant to this section. <br /> OBSERVATIONS: <br /> The biohazard waste containers utilized in the patient care area remain open during the day and are used as primary and <br /> secondary containers (see PHOTO 1). <br /> CORRECTIVE ACTIONS: <br /> Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or"BIOHAZARD" on the lid and on <br /> the sides so as to be visible from any lateral direction. Ensure biohazard waste is placed in a primary container lined with a <br /> biohazard bag with a tight fitting lid, in the patient care area. The lid must be closed when the container is not being used. <br /> Provide EHD with photographic evidence of correction. <br /> 30. HSC 118310 -Designated accumulation area shall be secured and marked with proper signage. <br /> OBSERVATIONS: <br /> The accumulation area sign does not meet the requirements of the Medical Waste Management Act (see PHOTO 2). <br /> CORRECTIVE ACTIONS: <br /> A designated accumulation area shall be secured by use of locks on entry doors, gates, or receptacle lids and marked with <br /> warning sign, "CAUTION—BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED PERSONS KEEP OUT", in <br /> English and in another language determined to be appropriate by the infectious control staff or LEA. Replace the current sign <br /> with the exact wording as mentioned above and provide photographic evidence to the EHD. <br /> Correct all violations and provide evidence of correction to the EHD <br /> by September 10, 2025. <br /> Notes: <br /> 1. Inspection report emailed to Jackson Felt, Center Manager, at Jackson.felt(cDCSLplasma.com. <br /> 2. Stericycle is the medical waste hauler for the facility. The frequency of medical waste removal from the site has increased to <br /> twice a week. <br /> 3.
