This registered large medical waste generator does not maintain individual treatment record3 and shipping and tracking <br /> documents for 2 years. <br /> -A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, and <br /> shipping and tracking documents for all untreated medical waste shipped offsite for treatment fcr 2 years. HSC117975(a)– <br /> Treatment records were not maintained for all untreated medical waste. Facility administrator provided documents <br /> during the inspection.–Corrected on Site <br /> 24. Pharmaceutical waste is not segregated for storage. <br /> - Nonradioactive and non-RCRA pharmaceutical wastes, that are regulated as medical waste, shall be placed in a container or <br /> secondary container labeled with the words"HIGH HEAT OR INCINERATION ONLY' on the lid and sides, so as to be visible <br /> from any lateral direction. HSC 118275(a)(6)(B)–The pharmaceutical container was not labeled correctly. Ensure all sides <br /> and the lid of the container is labeled with the words "HIGH HEAT OR INCINERATION ONLY." <br /> 25. Biohazard bag is not placed in a rigid container that is leak resistant, tightly lid, clean, in good repair and appropriately <br /> labeled. <br /> -Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be kept <br /> clean and in good repair. Container shall be labeled with the words "Biohazardous Waste" or"BIOHAZARD" on the lid and on the <br /> sides so as to be visible from any lateral direction. HSC 118280(c)–The gray container in the isolation room did not have a <br /> tight fitting lid. Provide a tight fitting lid for the biohazardous container. <br /> 30. The designated accumulation area is not secured or marked with proper signage. <br /> -A designated accumulation area shall be secured by use of locks on entry doors, gates, or receptacle lids and marked with <br /> warning sign, "CAUTION—BIOHAZARDOUS WASTE STORAGE AREA—UNAUTHORIZED PERSONS KEEP OUT', in English <br /> and in another language determined to be appropriate by the infectious control staff or LEA. HSC118310–The designated <br /> accumulation area did not have a warning sign in English and in another appropriate language. Update the warning <br /> sign and provide photographic evidence to the EHD. <br /> 31. The biohazard bag that is used to collect medical waste within a facility shall be manufacturer certified to meet the ASTM <br /> D1709 dart drop test, provided that when the bag is prepared for transport offsite, it is placed into a USDOT-approved container <br /> lined with a biohazard bag that is ASTM D1709 and ASTM D1922 certified. HSC 117630–The secondary container was not <br /> lined with a biohazard bag. Ensure the secondary container is lined with a biohazard bag when the container is in use. <br /> Correct all violations and provide evidence of correction to the EHD <br /> by March 13, 2023. <br /> Reinspecion on/about: A reinspection fee of$156 per hour may be charged.
