All other repairs must have a permit submitted to and approved by <br />the EHD prior to beginning work. <br />CORRECTIVE ACTION: Immediately obtain a permit from the EHD . A "Failure to Obtain a Permit" penalty applies. <br />This is a repeat violation, Class II. <br />302 <br />CCR 2630(d), 2641(a) Leak detection equipment not installed/maintained/located to detect leaks at earliest <br />opportunity. <br />OBSERVATION: According to the 10/12/2023 designated operator's month inspection report, the Bravo chain in <br />UDC 7/8 was broken and not located to detect a leak at the earliest opportunity. The broken chain was not repaired <br />until 10/26/2023. <br />REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br />possible opportunity. <br />CORRECTIVE ACTION: Ensure that all monitoring equipment is maintained and located to detect a leak at the <br />earliest opportunity. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />An inspection report was provided to the facility operator on the day of inspection. The report was amended <br />and replaces the initial report.
