Hicks : <br /> San Joaquin County ( County ) Department of Public Works ( Public Works ) staff has reviewed your <br /> Application for Services for Assessor ' s Parcel No . (APN ) 015 -450 - 11 ( 18911 North Lilac Street , <br /> Woodbridge ) from Mokelumne Acres Maintenance District ( District) . The parcel is currently outside of <br /> the District boundaries ; therefore , the District will serve the APN 015-450- 11 containing three ( 3 ) <br /> existing single residential dwellings and six ( 6 ) new single residential dwellings with water and storm <br /> drain services subject to the conditions outlined in this letter . <br /> 1 . Applicant shall initiate the annexation of the subject parcel into the District following the steps <br /> below ( Development Title Section 9 - 1100 . 4 ) : <br /> a . Submit to Public Works the annexation processing fee of $ 1 , 000 . 001 , the appropriate <br /> water service buy - in fee of $ 19 , 260 . 42 , and the storm drain buy- in fee of $ 8 , 157 . 09 , as <br /> well as the Area of Benefit No . 7 for storm drainage in the amount of $ 1 , 172 . 38 ( one <br /> check for the total amount of $ 29 , 589 . 89 made payable to San Joaquin County <br /> Treasurer) . <br /> b . Submit to Public Works the County Surveyor map and legal description processing fee of <br /> $ 1 , 000 . 001 ( make check payable to San Joaquin County Surveyor' s Office ) . <br /> c . Submit to Public Works the most current grant deed of the subject parcel containing the N <br /> title - holder name ( s ) and description of the subject property . <br /> d . Submit to Public Works the State Board of Equalization filing fee of $ 350 . 001 ( make <br /> check payable to State Board of Equalization ) . CD <br /> 2 . Applicant shall satisfy each of the applicable requirements below prior to obtaining water service C\J <br /> after completion of the annexation ( Development Title Section 9 - 1120 ) : <br /> a . Obtain an Encroachment Permit from Public Works prior to any work commencing in the <br /> County rlght- of-way or applicable easements . <br /> b . Install a water service connection ( s) , including water meter and appurtenances . An <br /> improvement plan of the proposed installation shall be submitted to Public Works for <br /> review and approval . <br /> c . Obtain a permit from the County Environmental Health Division if an existing well is <br /> to be destroyed .
