The following is an itemized list of underground storage tank violations that have not been addressed for GRANT LINE SHELL as of May05 , 2023 . Open violations from May 26 , 2022 inspection Violation #203 - Failure to maintain monitoring records for 36 months and maintenance records for 60 months. OBSERVATION Maintenance and monitoring records for the last three years were not found on site . Monitoring records include : ( 1 ) date and time of all monitoring or sampling ; (2 ) monitoring equipment calibration and maintenance records ; ( 3 ) results of any visual observations ; (4 ) results of sample analysis performed a lab or in the field ; ( 5 ) logs of all readings of gauges or other monitoring equipment, ground water elevations , or other test results ; (6 ) results of inventory readings and reconciliations . CORRECTIVE ACTION : Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site and submit copies to the EHD . ❑ This violation was corrected ❑ This violation will be corrected by (date) : ❑ Supporting documents included Describe actions taken or will be taken to correct violation : Violation #209 - Failure to maintain a copy of the " Facility Employee Training Certificate" on-site . OBSERVATION The Facility Employee Training Certificate dated 12/23/2021 : - does not have " Date of Assuming Responsibility as a Facility Employee" dates listed in the column - does not have years listed in the " Initial Training Date" column - has two employees listed as their initial training dates as " 5/ 13/2022" which is after the date of training " 12/23/2021 " REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if after October 1 , 2018 , or within 30 days of hire if prior to October 1 , 2018 . The Facility Employee Training Certificate shall contain accurate information for all employees trained . CORRECTIVE ACTION : Update the 12/23/2021 Facility Employee Training Certificate to contain the accurate information and have a separate Facility Employee Training Certificate for the 5/ 13/2022 training .
