OBSERVATION: Repair Secondary containment testing was performed on 12/14/2022 and a test report was submitted on 1/26/2023, 14 days late. REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. This is a minor violation. 208 CCR 2716(a-e)Failure to comply with the designated operator inspection requirements. 1). OBSERVATION:Work orders related to liquid removed from the fill sumps were not attached to the designated operator inspection reports of 4/20/23 and 5/16/23 or found on site. REGULATION GUIDANCE: The designated operator visual inspection report shall include: -a copy of documentation demonstrating action taken in response to each compliance issue identified during the previous inspection -a copy of the alarm history since the previous inspection -a copy of documentation demonstrating action taken in response to each alarm since the previous inspection -a list of each alarm since the previous inspection for which there is no documentation of the alarm condition and action taken in response CORRECTIVE ACTION: Ensure all required documents are attached to future designated operator inspection reports. ------------------------------------------------------------------------------------------------------------------------------- 2). OBSERVATION:The designated operator failed to note in section V follow up action taken to correct issues listed in section III, report dated 4/20/23 and 5/16/2023. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. Overall Inspection Comments: An inspection checklist was provided to the facility operator on the day of inspection. A complete report was finished on 6/6/2023 and replaces the initial checklist.
