Environmental Health Department <br />Underground Storage Tank Program Inspection Report <br />Date: <br />May 25, 2023 <br /> Facility Address: <br /> 3250 W HAMMER LN, STOCKTON <br /> Facility Name: <br /> AM PM HAMMER/I5 FOOD #83113 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />-The designated operator documented visual inspections of sumps and UDCs but didn’t answer the questions about <br />sensor locations. <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br />testing and maintenance for the UST system have been completed and shall check all required items on the report. <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br />all of the required information on the reports. <br />This is a repeat violation, Class II. <br /> 210 CCR 2715(c) Failure to provide initial training, train every 12 months, and have a trained employee present. <br />OBSERVATION: The designated operator employee training was not performed for all employees. <br />REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br />maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br />individual performs the duties of a facility employees. The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility’s best management practices <br /> 2. Employee’s role with regard to monitoring equipment as specified in the facility’s monitoring plan <br /> 3. Employee’s role with regard to spills and overfills as specified in the facility’s response plan <br /> 4. Name of the contact person(s) for emergencies and monitoring equipment alarms <br />Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br />evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least <br />one of the facility employees present during operating hours shall have current training. <br />CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on <br />site, and submit a copy of the training records to the EHD. <br />This is a repeat violation, Class II. <br />Overall Inspection Comments: <br />***The monitoring system certification is not complete. The Bravo float and chains were not tested. A current <br />Bravo certification was unavailable during the inspection. Immediately schedule float and chain test with a 48 <br />hours notification to the EHD. All EHD staff associated with a re-inspection will be billed at the current hourly <br />rate. <br />Note: Section 6 of the designated operator visual inspections were not signed by the UST owner/operator. <br />Ensure all future designated operator inspections are signed. <br />An inspection checklist was provided to the facility operator on the day of inspection (5/25/2023).
