Environmental Health Department Underground Storage Tank Program Inspection Report Date: May 25, 2023 Facility Address: 3250 W HAMMER LN, STOCKTON Facility Name: AM PM HAMMER/I5 FOOD #83113 SUMMARY OF VIOLATIONS (CLASS I, CLASS II, or MINOR - Notice to Comply) RemarksItem # -The designated operator documented visual inspections of sumps and UDCs but didn’t answer the questions about sensor locations. REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports. This is a repeat violation, Class II. 210 CCR 2715(c) Failure to provide initial training, train every 12 months, and have a trained employee present. OBSERVATION: The designated operator employee training was not performed for all employees. REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees. The training shall include, but is not limited to: 1. Operation of the UST system in a manner consistent with the facility’s best management practices 2. Employee’s role with regard to monitoring equipment as specified in the facility’s monitoring plan 3. Employee’s role with regard to spills and overfills as specified in the facility’s response plan 4. Name of the contact person(s) for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on site, and submit a copy of the training records to the EHD. This is a repeat violation, Class II. Overall Inspection Comments: ***The monitoring system certification is not complete. The Bravo float and chains were not tested. A current Bravo certification was unavailable during the inspection. Immediately schedule float and chain test with a 48 hours notification to the EHD. All EHD staff associated with a re-inspection will be billed at the current hourly rate. Note: Section 6 of the designated operator visual inspections were not signed by the UST owner/operator. Ensure all future designated operator inspections are signed. An inspection checklist was provided to the facility operator on the day of inspection (5/25/2023). A complete report was finished on 5/25/2023 and replaces the initial checklist.
