SAN JOAQUIN E <br /> Food Program Official Inspection Report <br /> Facility Name and Address: BON APPETIT MARKET PLACE, 901 PRESIDENTS DR, STOCKTON 95211 <br /> #23 Rodents, Insects or Animals Inside Facility MAJOR <br /> OBSERVATIONS:Observed the following: <br /> -One dead cockroach on the ground near the three compartment sink <br /> -Observed four live cockroaches under the two prep coolers in the deli area <br /> -Observed five dead cockroaches under the two prep coolers in the deli area <br /> -Observed one live cockroach under the cooking equipment in the grill area <br /> -Observed two baby cockroaches on the ground to the right of the cooking equipment in the grill area <br /> -Observed one live cockroach inside a hole in the wall to the right of the cooking equipment in the grill area <br /> -Observed one live cockroach under the hand sink at the back wall <br /> -Observed one live cockroach in cabinet below panini press <br /> -Observed a live baby and an adult cockroach on the floor under the ware washing hand sink <br /> -Observed a live roach on the heater of the large dishwasher <br /> -Observed baby roaches coming out from under deli display case cooler, a live adult on the inside behind the filter and <br /> several dead roaches on the inside behind the filter <br /> Clean and sanitize areas immediately. Increase pest control measures. Discard any contaminated items immediately. <br /> CALCODE DESCRIPTION:Each food facility shall be kept free of vermin:rodents(rats,mice), cockroaches,flies.(114259.1, 114259.4, <br /> 114259.5) <br /> #8 Use of Time as a Public Health Control <br /> OBSERVATIONS:Time control logs were lacking for items in the hot hold at the Taqueria area. Beans were observed at <br /> 107 F and sausage was observed at 100 F. Per employee,food items are changed out every two hours. Food shall be <br /> marked or a log shall be kept when food is removed from temperature control and the four hour time limit has been reached. <br /> Alternatively, repair hot hold unit so that it maintains food at 135 F or higher. Correct today. <br /> CALCODE DESCRIPTION:When time only,rather than time and temperature is used as a public health control,records and <br /> documentation must be maintained(114000) <br /> #30 Food Storage/Display Properly Labeled <br /> OBSERVATIONS:Open bags of rice were observed in the dry storage room. Once bags are opened, store inside a <br /> cleanable container with tight fitting lid. Correct today. <br /> CALCODE DESCRIPTION:Food shall be stored in approved containers and labeled as to contents. Food shall be stored at least 6" <br /> above the floor on approved shelving. (114047, 114049, 114051, 114053, 114055, 114067(h), 114069(b)) <br /> #33 Nonfood Contact Surfaces Clean <br /> OBSERVATIONS:Two hoods in the downstairs kitchen and the lair hood were observed with dust and grease build-up. <br /> Clean hoods within two weeks. <br /> Observed heavy grease accumulation and food debris on the sides and below cooking equipment. Clean and sanitize <br /> cooking equipment within three days. <br /> CALCODE DESCRIPTION:All nonfood contact surfaces of utensils and equipment shall be clean.
