COMPLAINANT STATES SHE INFORMED AN EMPLOYEE WHO ALLEGEDLY PICKED UP <br /> THE ROACH PUT IT ON THE GROUND AND SMASHED IT. COMPLAINANT STATES THAT THE EMPLOYEE THEN <br /> WENT TO TAKE CARE OF OTHER CUSTOMERS AND BEGAN TOUCHING CLEAN UTENSILS WITHOUT WASHING <br /> THEIR HANDS. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> #23 Rodents, Insects or Animals Inside Facility <br /> OBSERVATIONS:Observed two dead cockroaches on top of the dish machine in the kitchen. Clean and sanitize this area <br /> today. <br /> CALCODE DESCRIPTION:Each food facility shall be kept free of vermin:rodents(rats,mice), cockroaches,flies.(114259.1, 114259.4, <br /> 114259.5) <br /> #29 Toxic Substances Properly Identified/Stored/Used <br /> OBSERVATIONS:Observed a can of Hot Shot fogger underneath the hot food wells at the salad bar side. Discontinue use <br /> of household insecticides. Use commercial grade/food safe products only. <br /> Fogger can was discarded. <br /> CALCODE DESCRIPTION:All poisonous substances, detergents, bleaches, and cleaning compounds shall be stored separate from food, <br /> utensils,packing material and food-contact surfaces. Per Kitchen Manager,the facility is serviced at least once a <br /> month (if not more frequently)by Matrix Pest elimination.The most recent dates that the facility has been serviced are: <br /> 9-10-23, 8-22-23, 8-9-23, and 7-30-23.
