Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: C00055250 Site Location: 6301 W BANNER RD <br />Activity Summary <br />Activity Date <br />09/23/2022 Received a page from the front counter. Called Leanne with Valley <br />Communications (209-236-8105) to gather more information. The spill was on <br />going and located in the parking lot of Carl's Jr. at 6301 West Banner Street, Lodi, <br />CA. Two ruptured fuel saddle tanks are releasing fuel into the parking lot and the <br />private storm drain system. I arrived on site at approximately 3:00pm and met with <br />Jaime Ramirez, the Battalion Chief for Woodbridge Fire. Jaime informed me that <br />the saddle tanks on the bobtail truck were ruptured in Sacramento County <br />yesterday (9/22) because the bobtail truck hit the rap on the semi-truck it was <br />being transported on. Sacramento County patched up the saddle tanks and the <br />responsible party continued on their drive. Per Jaime, the drivers parked in the <br />Carl's Jr. parking lot at approximately 8:38pm as seen on camera footage. Leak <br />was reported at around 2pm and it is estimated to be approximately 15 gallons. <br />The responsible party is Wagoneers Trucking based out of Billings, Montana. <br />Spoke with David Green (406-248-1919) who forwarded me to their environmental <br />safety contact, Tim Neredith (713-545-2998). Tim stated that they would be hiring <br />out ACT Environmental and provided me with their primary contact. I called <br />Lauren Johnson (863-712-4404) with ACT who informed me that they contracted <br />out Ancon Marine Services to perform the clean up and they should be arriving on <br />site in approximately 2 hours. I provided them with the after hours inspector <br />contact number and let Lauren know that she should contact her. Lauren also <br />provided me with her email <lauren.johnson@a-c-t.com> <br />Recorded by <br />GARCIA-MEJIA <br />GARCIA-MEJIA 09/26/2022 Received CalOES spill report #22-5612 for incident that occurred on 9/23/22. <br />Generated prop 65 and provided it to Kim to be mailed out. Generated emergency <br />response record and printed the complaint history and parcel viewer information. <br />Saved photos to the public drive and created a photo log. <br />Issued inspection report stating the following <br />"The responsible party retained Ancon Marine Services to conduct clean up. Two <br />55-gallon drums of waste were generated, <br />labeled and left on site pending disposal. Private storm drain was cleaned up with <br />water and vacuumed out. <br />Please submit analytical results and final disposal records to the EHD when they <br />are received. <br />Report was emailed to Lauren Johnson with ACT Environmental at <br />lauren.johnson@a-c-t.com" <br />09/27/2022 Received an email from Lauren Johnson <lauren.johnson@a-c-t.com> confirming <br />she received the notice to abate. Laurent asked what follow up was needed and I <br />informed her that I would need a final signed copy of the manifest for the waste <br />that was generated. <br />11/08/2022 Emailed Lauren Johnson for an update on the disposal of the waste generated. <br />Lauren emailed me a copy of the manifest that did not include the final signature. <br />Received a call from Lauren and we discussed what was missing in the manifest. <br />She provided me with a copy of the final signed manifest. <br />11/09/2022 Printed final manifest received from Lauren <lauren.johnson@a-c-t.com>.
