NO <br />EVACUATION OCCURRED, NO PERSONNEL EXPOSED EQUIPMENT WAS REPAIRED AND LEAK WAS MITIGATED A <br />APPROXIMATELY 0830. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />NOTES <br />OBSERVATION' APPROXIMATELY LESS THAN ONE POUND OF ANHYDROUS AMMONIA WAS RELEASED FROM <br />CHILLER#1 FROM AREA C, DUE TO A SEAL DEGRADING ON THE GAUGE RELEASE OCCURRED APPROXIMATELY K <br />1:15AM ON 7/27/2021. SEAL WAS REPAIRED AT APPROXIMATELY8:30. EQUIPMENT WAS NOT IN USE DURING THE <br />RELEASE. NO EVACUATION OCCURRED, AND NO PERSONS WERE EXPOSURE OR INJURY OCCURRED DUE TO <br />RELEASE. EQUIPMENT IS BACK TO OPERATING CONDITION. INCIDENT INVESTIGATION IS IN PROGRESS. <br />CORRECTIVE ACTION: SUBMIT: <br />- INVESTIGATION REPORT DETAILING THE ROOT CAUSE OF THE INCIDENT <br />RECOMMENDATIONS RESULTING FROM RELEASE INCIDENT <br />WORK ORDER INVOLVING REPAIRS RESULTING FROM RELEASE <br />PHOTOS TAKEN DUE TO RELEASE <br />SUBMIT DOCUMENTS TO THE EHD BY 8/26/2021 FOR REVIEW. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
