A body art facility shall be separated from any residential areas used for sleeping, bathing, or meal preparation and shall not <br />share a common entrance or toilet facility with residence. The toilet facility must have a sink with hot and cold water, <br />containerized liquid soap, and single -use paper towels that are dispensed from awall-mounted, touchless dispenser. HSC <br />119314 —The men's and women's restrooms did not have hot water and the women's restroom did not have paper <br />towels in the dispenser. Restock paper towels in the dispenser. Ensure the hot water issue is resolved as soon as <br />possible and contact EHD for a reinspection. <br />32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br />The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br />floor, from nail and hair activities and be separated from all business not related to body art. Approval of co -located body art <br />facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to pertorm a procedure. Each <br />procedure area shall have lined waste containers. HSC 119314 & 119317 —The garbage containers in Breanna's and <br />Crystal's procedure rooms did not have liners. Crystal corrected the violation on site. Line Breanna's garbage container <br />with a liner and provide photographic evidence to the EHD. <br />35. Operation and employee training records shall be maintained for 3 years and made available for inspection. A written <br />Infection Prevention and Control Plan (IPCP) shall be maintained, followed and updated. HSC 119313 —The IPCP was not on <br />site and training records were not available. Ensure IPCP is on site at all times and practitioners are trained on an <br />annual basis or when the IPCP is updated. Submit a copy of the training records to the EHD. <br />Unless otherwise noted, submit all corrections to the EHD by May 27, 2021. <br />Notes: <br />1. Breanna will pay her permit fee within the next couple of weeks. <br />3. Sharps Assure is used for sharps waste disposal by all practitioners. Ensure sharps waste is disposed of once a year or when <br />container is full. <br />4. Crystal does not intend tb practice body art until August 2021. <br />5.
