COMPLAINANT STATES THAT HER HUSBAND PICKED UP THE PIZZA AT 6:00 PM, HE HAD A SLICE OF THE <br /> PIZZA AND FOUND IT WAS COLD. COMPLAINANT STATES THAT ACCORDING TO THE RECEIPT THE FACILITY <br /> MADE THE PIZZA AT 3:30 PM. COMPLAINANT CALLED THE FACILITY TO ASK WHY THE PIZZA WAS COLD AND <br /> WAS ALLEGEDLY TOLD THAT THE PIZZA WAS MADE AND THEN LEFT OUT ON THE COUNTER FOR 2 HOURS. <br /> COMPLAINANT ALSO STATES THAT 12 HOURS AFTER EATING HER HUSBAND BEGAN TO FEEL SICK WITH <br /> SYMPTOMS OF NAUSEA AND VOMITING." <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Per GM, the pizza was made early and did sit out for a few <br /> hours. GM states that the employee did not look at the pick-up time on the order tag.
