Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COY39LETL Site Location: 1495 E PESCADERO AVE <br />Activity Summary <br />Activity Date Recorded by <br />07/30/2022 6:11 am-Called Juan (Reddaway) at 209-614-5880 for an update on clean-up. BAKER <br />Clean-up will be done in one and a half hours. 6:20 am-Left for site. Arrived on <br />site at 7:00 am. Clean-up of trailer is completed. No storm drains or waterways <br />were impacted. Colorant called Mulch Magic Chery Brown. Spill in trailer was <br />cleaned up with absorbent. Approximately 50 gallons of colorant was left in tote. <br />Colorant was placed in 55 gallon drum for disposal. Plastic tote was cut up and <br />placed in 55 gallon drums for disposal. Waste will remain on site pending disposal <br />as hazardous waste. Spill on concrete impacted an approximate area of 10x10. <br />Left site at 7:25 am for another complaint. <br />9:07 am-Arrived back on site to verify clean-up. (worked on paperwork for another <br />complaint from 9:07 am-10:51 am while clean-up was in progress). <br />10:51 am- Generated complaint, prop 65, Emergency response record. Started <br />OIR and photo log. <br />Emailed OIR to Ruben Byerley (CERS Environmental) at <br />ruben.byerley@yrcfreight.com <br />12:45 pm -Clean-up is complete. Five 55 gallon drums of liquid waste and ten 55 <br />gallon drums of solid waste was generated from the clean-up. Left site. <br />6:11 am - 7:25 am <br />10:51 am - 12:45pm <br />1:10pm - 1:51pm <br />08/01/2022 Updated complaint information. Completed site map and photo log. BAKER <br />Disposal records will be verified on next routine inspection for hazardous wastes <br />(PRO527431). <br />Turned in prop 65 to Lead Sr. REHS for review. <br />Abated complaint as 01-Field Response - Violations cited and corrected.
