OBSERVATION:An approved current copy of the response plan was not available for inspection. REGULATION GUIDANCE:A copy of this plan shall be accessible on site at all times. CORRECTIVE ACTION: Immediately locate and retain a copy on site. This is a minor violation. 306 HSC 25291(e)Failure to remove, analyze, and properly dispose of water in secondary containment(pre-Jul 2003). OBSERVATION: Liquid was observed in the 91-grade submersible turbine pump(STP)sump. REGULATION GUIDANCE: If water could enter into the secondary containment by precipitation or infiltration, it must be removed and disposed of properly. CORRECTIVE ACTION: This was corrected on site. The service technician removed approximately 1 quart of liquid from the 91 STP sump. Ensure that all sumps and annular spaces are maintained free of liquid. This is a repeat violation, Class II. 317 CCR 2636(f)(2) Failure of the line leak detector to detect a 3.0 gph leak at 10 psi and restrict or shut off flow. OBSERVATION: The 87-grade(North)VMI line leak detector failed to detect a leak when tested. REGULATION GUIDANCE:All line leak detectors shall be capable of detecting a 3-gallon per hour leak at 10 psi. CORRECTIVE ACTION: This was corrected on site. The service technician adjusted the leak detector, retested it, and it passed. This is a Class II violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 5/5/2023. To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of having them sign.
