Certificate of registration and health permit shall be posted in a conspicuous place. HSC 119306 and 119312 —The facility <br />permit and Kevin Murphy's permits were expired. Ensure the current permits are posted. Send photographic evidence <br />to the EHD. <br />37. A body art facility shall not conduct business without a valid health permit. A person shall not perform body art without a valid <br />registration and a permitted facility. A body art facility shall notify the LEA in writing within 30 days of resignation, termination of a <br />practitioner. Anew hire or apprentice must be registered the EHD prior to performing body art at the facility. HSC 119312 & <br />119306— Scott Bender has not paid the current registration fee, he does not currently possess a valid permit from the <br />EHD. Scott Bender is prohibited from performing body art immediately. <br />Correct all violations and submit evidence of corrections to the EHD <br />by May 16, 2022. <br />Notes: <br />1. Inspection report emailed to Shop Manager, Galen Reyes, at emeraldtattooCrilgmail.com. <br />2. Stericycle is the designated sharps waste disposal company utilized by the facility. The last disposal was on 2/8/22. <br />3. Practitioners were trained on the Infection Prevention and Control Plan on 1/18/22. <br />4.
