OBSERVATION: The designated operator inspections for the following months were performed more than 30 days after the previous inspection: - Initial inspection on 5/1/2022, next inspection performed on 6/6/2022 (36 days)Report states that the inspection was conducted late due to COVID-19. - Initial inspection on 8/5/2022, next inspection performed on 9/5/2022 (31 days) - Initial inspection on 1/2/2022, next inspection performed on 2/2/2022 (31 days) REGULATION GUIDANCE: The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time frames. This is a repeat violation, Class Il. 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION: The Facility Employee Training Certificate dated 2/2/2023 lists incorrect initial training and assumed responsibility dates. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1, 2018, or within 30 days of hire if prior to October 1, 2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. CORRECTIVE ACTION: Update the Facility Employee Training Certificate to contain the accurate information and submit a copy of the updated form to the EHD. This is a minor violation. Overall Inspection Comments: Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 5/6/2023. To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of having them sign.
