3.0 INSPECTION RESULTS <br />The following results are the summarization of a field checklist <br />that was utilized during the inspection of T-631 3134A <br />3.1 Foundation: <br />3.1.1 The foundation is constructed of a concrete pad. The containment structure <br />appears the meet the capacity requirements recommended by NFPA 30. <br />3.1.2 The concrete pad has heaved along the centreline of the storage tanks <br />running in a East/West plane. The heaving has caused the anchor chair fasteners <br />to decouple from their embedment in the concrete. It was noted that additional <br />chime hold down fastening was added to the North chime.The containment <br />structure consists of a concrete wall and an underground piping system designed to <br />increase and provide suitable capacity for containment. <br />3.1.3 The containment structure has a few hairline cracks through out the <br />circumference as well as larger cracking. Recent heavy rains caused the <br />containment system to fill with 5-8" of rainwater. No evidence of a loss of <br />containment was noted. <br />3.1.4 The tank bottom-to-foundation gap has no sealant coverage to prevent <br />rainwater ingress under the tank bottom. <br />3.2 Shell: <br />3.2.1 The tank shell external surface has an overall smooth surface profile. No <br />denting, bulges, distortions or abnormalities noted. <br />3.2.2. There were several small areas of localized corrosion coating failure. <br />3.2.3 Recent storms and rain has caused containment system to fill and leave a <br />film on the lower coated surfaces of the shell. <br />3.2.4 There are some spotted areas of the external surface of the shell where the <br />coating has been chipped or failed to the base metal where surface oxidization <br />corrosion is active especially on the spiral stairway." <br />3.3 Nozzles: <br />3.3.1 The shell nozzles are in satisfactory mechanical condition. No evidence of <br />leaks or valve failure. <br />3.3.2 The tank nozzles have some scattered coating failure. <br />3.4 Roof: <br />3.4.1 Tank roof is coated and has some issues with external coating failure <br />throughout. <br />3.4.2 Roof mounted equipment includes a breathing valve and emergency vent <br />valve. Equipment was operating to set pressures and verified against mounted <br />analog pressure gauges. <br />3.4.3 The tank has been designed and built to meet the requirements of API 650, <br />Section 5.10.2.6 for Frangible roof. <br />3.5 Floor: <br />N/A <br />3.6 Ancillary Equipment: <br />3.6.1 Tank was grounded with cable to ground plate, all connections tight.
