Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COO1P6EIX Site Location: 530 W KETTLEMAN LN, LODI CA 95240 <br />Activity Date <br />Activity Summary <br />Recorded by <br />09/20/2022 Printed out the notice to abate, photo logs, 5104, complaint history, and began <br />working on the emergency response record. <br />GARCIA-MEJIA <br />09/21/2022 Completed the emergency response record and printed it. GARCIA-MEJIA <br />10/19/2022 Received copy of uniform hazardous waste manifest via email from Scott Smith GARCIA-MEJIA <br />(Scott.Smith@kohls.com) via email on 10/18/22. Manifest #016244336JJK. 4 <br />drums totaling 800 pounds were hauled off by Patriot Environmental. I printed out <br />the manifest and added it to the packet. Reviewed photos of manifests taken <br />during clean up. Manifest for the oily liquid #024363559JJK has not been received <br />as well as the non-hazardous manifest for solid debris. <br />10/20/2022 Emailed Scott inquiring about the disposal records which were noted during the <br />final clean up. Emailed him the following: <br />GARCIA-MEJIA <br />"Hi Scott, <br />Thank you for sending this over. During the cleanup there were 2 different <br />manifests that were generated which do not match the attachment. <br />-One is for non-hazardous waste (solid debris), 2 drum of 150 pounds with a date <br />of 8/13/22. <br />-The other is a Hazardous waste manifest (oily water) for 175 gallons. Manifest <br />#024363559JJK <br />Do you have final copies of those manifests?" <br />10/25/2022 Received the following email from clarification from Scott Smith, compliance <br />manager: <br />GARCIA-MEJIA <br />"Hi Gabriela, <br />Sorry for the wait and the confusion. Attached are the two final manifests for the <br />Solid and Liquid hazardous waste. <br />We were trying to figure out why you were reporting two drums but the manifests <br />showed 4, which caused some of the delay. So 2 drums of solid waste were <br />generated on the first day 8/10 and then another 2 were generated on 8/13. When <br />they got back to the storage site, Patriot decided to consolidate the 4 drums of <br />solid on a single hazardous waste manifest. I'm adding the two initial manifests to <br />show the connection. <br />Please let me know if you have any questions and if this closes the notice to <br />abate. Thank you!" <br />Scott provided me with a copy of the final signed manifest for the oily liquid and <br />pictures of both manifests mentioned in the email as well as the final oily solid <br />manifest. <br />I printed out all documents and emails and abated the complaint.
