Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COZDOT59Q Site Location: 1000 E ROTH ROAD, FRENCH CAMP <br />Activity Summary <br />Activity Date Recorded by <br />03/07/2022 An approximate 4-5 gallons of Adhesive UN1133 was released. Container ERNEST <br />UMXU251431 was holding the adhesive when it was released, spilling onto <br />container UMXU895912. Hazardous waste is to stay in the custody of Union <br />Pacific Railroad, and is pending disposal. <br />Received spill report indicating RP states that an unknown amount of adhesive <br />was released onto the concrete at rail yard from the rail car #UMXU251431, the <br />release has been stopped and contained, the contractor will e conducting clean <br />up. Spill report was received from Kristina Saetern EHD who indicated they will not <br />be able to visit the site during work hours. <br />Called environmental response management at 888 877 7267. Patty <br />environmental health dispatcher indicated 2 different rail cars were affected by the <br />adhesive spill. It was not indicated how much adhesive was released. <br />Arrived on site at approximately 5:30 pm checked in with Steven Zeehandelaar <br />Union Pacific switch manager, who gave permissions to observe the affected site <br />and to take photos. <br />Discussed spill with Jose Ochoa, US Ecology, Field Service Foreman II. <br />Discussed how he did not know the originate spill site. Also, the amount spilled is <br />unknown. Discussed pending clean up. <br />Discussed original site of spill with Steven Z. who indicated he could not direct me <br />to the original spill site. <br />Received page from Lenay Kubes. Called Lenay Kubes at 888 877 7261, and <br />discussed site disposition. She could not give a responsible party, or an amount <br />released. <br />Populated prop 65, notice to abate, and narratives. <br />Pending hazardous waste labeling on the 55 gallon black barrel generated on site. <br />Indicated by Jose 0. he would provide photos of labeling and documentation <br />resulting from the spill. <br />Left site at approximately 9pm <br />03/08/2022 Discussed update of clean up with Fred Ernest, REHS. Phoned Jose Oacha, Field SAETERN <br />Services Foreman ll 510-671-4691 of US Ecology, requested for photos of labels <br />on the drum. Per Jose, he will e-mail the photos with the description of the label <br />due to the photos are not legible. <br />03/08/2022 Discussed emergency response that occurred on 3/7 with Kristina Saetern EHD. <br />Transfered photos and notice to abate to Kristina S. Discussed response with <br />Elena Manzo EHD. Reviewed photos sent by Jose Ochoa <br /><Jose.Ochoa©usecology.com> .
