S A N J 0 A Q I I I Irl Environmental Health Department <br /> C o u N T AMENDED <br /> 4/4/2023 <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> AKB TRANSPORT INC 3845 S EL DORADO ST, STOCKTON December 28, 2022 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: The Hazardous Materials Inventory forms for Oxygen and Used lubricating oils were not complete. <br /> They listed obsolete hazard categories and inaccurate container sizes. <br /> REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br /> of the business plan. <br /> CORRECTIVE ACTIONS: This was corrected on site. <br /> OBSERVATION:Approximately 300 gallons of motor oil were found on site that have not been reported to CERS. <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION:A site map was not completed and submitted as part of the business plan. <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: This was corrected on site. <br /> This is a minor violation. <br /> FA0010498 PR0520388 SCO01 12/28/2022 <br /> EHD 19-01 Rev.12/06/2021 Page 3 of 6 Hazardous Materials Program OIR <br /> 1868 E.
