SANIAJOAQUIN cou N 1 Y <br />Creotr•csN 9—)ws hert. <br />Environmental Health Department <br />Time In: 1:25 pm <br />Time Out: 2:05 pm <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: JACK IN THE BOX #565 Date 03/29/2023 <br />Address 3506 W HAMMER LN, STOCKTON 95209 <br />Owner/Operator: Telephone: (510) 792-3393 <br />Program Element: 1600- FOOD PROGRAM Complaint #: C00055719 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />CUSTOMER WENT IN THROUGH THE DRIVE-THRU TODAY PER THE ATTACHED RECEIPT AT 1:07PM AND WHEN <br />COMPLAINANT HUSBAND OPENED TACO TO PUT TACO SAUCE INSIDE HE NOTICED WET SLIMY SUBSTANCE IN <br />THE TACO THAT HE IS VERY CONCERNED TO BE SOMEONE'S SALIVA. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />FOOD ITEM -- LOCATION --TEMP ° F -- COMMENTS <br />No Temperature Data Collected <br />COMMENTS AND OBSERVATIONS <br />Complaint inspection. No major violations. No re-inspection. <br />Facility is not aware of complaint. Tacos contain beef, slice of American cheese, salsa and shredded lettuce. Tacos shell <br />with beef is fried and then a slice of cheese, salsa and lettuce are added. Showed facility photos of taco with alleged saliva. <br />Facility said that the taco looks the same as always. Had facility show me the inside of the taco. Taco appeared to be the <br />same as the photos. Facility has cameras in the kitchen and manager will look at video from the date and time to verify <br />nobody spit in the taco. <br />Official inspection report was emailed to operator. <br />To minimize person-to-person contact, the signature of the person receiving the inspection report was not captured. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
