Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: C00055423 Site Location: 1430 S CHEROKEE LN <br />Activity Summary <br />Activity Date Recorded by <br />12/14/2022 Arrived on site and met with the Ed Sanchez, Security Guard, who stated Central SAETERN <br />Valley Waste Management arrived in around 6:00 AM on Tuesday and began <br />digging. Per Ed Sanchez, they placed the materials into a 5 gallon bucket. <br />Observed the affected area. No residue was observed on the gravel floor and the <br />surrounding area in the PG&E Storage Yard. <br />Met with Tony Serrano, Materials Handler, at the PG&E Storage Yard who stated <br />Central Valley Waste Management (CVWM) supervisor arrived on site around <br />7:10 AM to verified clean-up. A vacuum truck arrived to remove the remaining <br />residue. This was verified with Ricky, manager. <br />Phoned Fabian Speede, 209-275-5441, of CVWM to clarify what had occurred <br />over the last two days. According to Fabian, <br />Monday 12-12-2022, Butler's Cleaning Sweep send a vacuum truck to removed <br />the liquid with the coolant. Approximately 1.5 gallon of liquid was removed and <br />was taken to CVWM to be dispose of. <br />Tuesday 12-13-2022, CVMW arrived to clean the remaining residue. <br />Approximately 1/2 a bag of absorbent was placed onto the impacted area. <br />Approximately 1/2 gallon of contaminated absorbent and some gravel was <br />removed and placed into a 5 gallon bucket. This bucket was taken back to CVWM <br />for disposal into their hazardous waste drum. <br />CVMW sent Chris Romriell, Environmental Manager, to verify final clean-up. <br />Phoned Chris Romriell 209-471-7396. Per Chris he had the supervisor of Butler's <br />Clean Sweep, Albert, come out to the site to verify clean-up. They met with PG&E <br />manager Ricky. They verified the clean-up was completed. Currently waiting for <br />Butler's Cleaning Sweep disposal record and time line of event. <br />Phone Albert of Butler's Clean Sweep, 209-649-1093, who stated: <br />Monday 12-12-2022, they arrived around 4 PM before PG&E facility closed. They <br />vacuumed approximately 1 gallon of the coolant contaminated with the puddle. <br />This was taken to CVWM and disposed of into their oil-water separator. <br />Tuesday 12-13-2022, Butler's Clean Sweep went out with a vacuum truck and a <br />sweep truck. After vacuumThey removed roughly 1 gallon of absorbent and gravel <br />which was placed into a 5 gallon bucket. CVWM took the contaminated waste to <br />their facility. Per Albert, he does not know what had happened to the macro-lid and <br />did not observed the lid to be on site. <br />Received phone call from Fabian Speed, who stated the hazardous waste that <br />was generated from the site on both days was taken to CVWM facility. The liquid <br />from Monday was disposed into their oil-water separator and the waste from the 5 <br />gallon bucket was placed into their hazardous waste drum. Per Fabian, the <br />oil-water separator will be cleaned out and the drum will be picked up in a few <br />days. Requested for the disposal record once the waste have been removed. Per <br />Fabian, he does not know what had happened to the macro-lid. <br />Updated J. Alaniz, Lead REHS. <br />E-mailed Romriell, Christopher <cromriel@wm.com> and Speed, Fabian <br /><fspeed@wm.com> a photo of the Macro-lid containing the coolant, requesting for <br />the missing lid.
