IS 1 o5 Abated By: <br />Activity Activity Summary <br />Activity Date Recorded by <br />01/24/2023 (7:54pm-8:18pm)-The EHD received CalOES spill report #23-0726 for 10-15 GARCIA-MEJIA <br />gallons of diesel. The saddle bad on a semitruck was punctured due to unknown <br />reasons resulting in the release of the listed substance onto the asphalt and <br />concrete only. No waterways or storm drains were impacted. Sand, absorbent and <br />pads were used to mititgate the release. The responsible party contracted Ancon <br />Environmental to conduct clean up. Clean up was completed the same day. <br />Approximately 1 55-gallon drum of waste was generated and stored on site for <br />later disposal. I received photos of the clean up via text from Chase Maeser. <br />01/25/2023 Generated complaint and completed dailies. Saved photos to the file. Chase <br />provided me with a contact number for New Prime Inc., the responsible party <br />(417-521-3602) and he confirmed that the drum of waste generated was stored in <br />the facility's hazardous waste storage area. I spoke with the dispatcher there and <br />they told me to call the safety department at 1-800-491-1253. Spoke with John <br />Hardin who was not able to find a complaint on his end. I provided John with the <br />truck number and he was able to confirm that it is one of their vehicles. John <br />provided me with his direct number (417-521-3931) and a good mailing address <br />as well as an email address <safe@primeinc.com>. He also asked that I include <br />the reference number 207698 in the subject line of any emails. <br />GARCIA-MEJIA <br />01/26/2023 Issued a complaint inspection report and emailed it to <safe@primeinc.com>. GARCIA-MEJIA <br />Approximately 10-15 gallons of diesel were released due to a ruptured saddle <br />tank. No waterways or storm drains were <br />impacted. The responsible party retained Ancon Environmental to conduct clean <br />up. Clean up was completed the same day, <br />1/24/23. Approximately 1 55-gallon drum of waste was generated and was stored <br />on site pending disposal. Please provide the <br />EHD with a copy of the final disposal record. <br />03/10/2023 Reviewed narratives. I emailed <safe@primeinc.com> asking if the waste was GARCIA-MEJIA <br />disposed of already and if so to please provide me with a copy of the final signed <br />manifest.
