NSO <br />Heather directed me to call Jacob 812 629 1640 because she was already off <br />duty. Called Jacob and he stated that clean up had been completed about three <br />hours ago using absorbents, and approximately 20 pounds of waste was <br />generated from the cleanup operation. I let him know that EHD will need a copy of <br />the disposal records and waste profile. Jacob accepted to send disposal records <br />and photos to Fred. On site to verify cleanup. Observed impacted area/concrete <br />with stains and discus cleanup with the on duty Flying J supervisor Pete Ness. <br />Clean was not looking adequate to me. Called Jacob 812 629 7018) and discus <br />cleanup. Jacob stated that when the cleanup was completed the crew discus it <br />with the on duty Flying J Supervisor Joshua Cardona and Joshua admitted the <br />cleanup was ok and accepted it as clean. <br />10/03/2022 Cal OES Control #:22-5821 ERNEST <br />Received spill report at approximately 1:00 pm. Spill report indicated A release <br />was spotted from a fuel injector <br />on a semi truck. The release is stopped and contained. No waterways or storm <br />drains were impacted. Clean up is en route through a contractor. Called <br />Discussed spill with John Alaniz. Traveled to site, and got in contact with Joshua <br />Cardona. found a spill approximate 5-15 gallon spill, covering an area of about a <br />10*20 square area. The spill location is in the semi truck parking near the rear <br />west side of the facility. Spill did not affect storm drains, and no diesel went to the <br />stormceptor. Left site after taking photos and sent a notice to abate to the <br />responsible party at JB Hunt. <br />10/04/2022 Completed narratives, reviewed email discussing complaint. ERNEST <br />10/06/2022 Updated and turned in prop 65s, completed amended notice to abate as per John ERNEST <br />Alaniz and sent amended notice to abate to matt.pittman@jbhunt.com responsible <br />party environmental contact. <br />Received prop 65 with notes to correct.
