I do not have Kristen's email
would you mind forwarding or giving me her email.Andrew will be sending over the RTC document also.
I apologize,we just found out the facility manager left his position, and did not leave information or the document
information for the managers onsite. We are in the process of working with the Manager of this location and getting
them back into compliance and knowing where the documents are able to be retrieved 24/7 from our electronic system.
Item 202-Copy attached Permit, Original was sent to Mgr. in January to post. Copy is kept electronic, and mgr. was
advised to post a copy onsite and at Veeder root panel
Item 205-Copy in Cers and available to mgr.24/7 electronically, copy attached
Item 206- Emergency Response Poster and ERP Map attached—available 24/7 electronically, Sent to mgr.to post onsite
Item 208—Inspections—This was addressed on our 2022 inspection, Reason for this was it was during COVID our DO
was extremely ill in the hospital with covid, we attempted other DO's during this time and there was a shortage of
technicians. This exact item was addressed our RTC in 2022. We have reiterated with our DO"s that all forms are signed
monthly and our new mgr.to sign monthly.
Item 209-210 Again, unfortunately the facility manager left and did not schedule the class C DO led training. This has
been scheduled for 7/11 and 7/19 DO training, all forms will be signed and updated accordingly during this time. I will
send the forms over after completion.
Item 307—Repaired onsite-We sent a technician to investigate, parts are on order for new lid/seals to help address the
water intrusion.
Item 310-Contacted Jones Covey to investigate and replace spill bucket and apply for Cupa permit.
2
