Attached is the inspection report for the inspection conducted on April 14th. I spoke <br />with my supervisor about the spore tests and it was decided that the facility may not <br />use the sterilizer until a passing spore test is provided. Going forward, a spore test <br />will be required to be submitted on a monthly basis to me for the next 6 months (it is <br />noted in the inspection report). I will need the following corrections from you: <br />1. Once the autoclave passes the spore test and the facility begins using the <br />autoclave again, then I will need evidence of proper labeling of the instrument <br />packages. <br />2. Updated sterilization log. <br />3. Passing spore test. <br />4. Evidence, such as a manifest, of sharps waste disposal. <br />5. Photographic evidence showing the sharps waste bin is labeled properly. <br />6. A current BBP Certificate from Katrina. <br />7. Updated tattoo consent form with the "description of what the client should <br />expect following the procedure." <br />8. Updated permanent makeup aftercare instructions "indicating when to seek <br />medical attention." <br />9. Updated Infection Prevention and Control Plan indicating Stericycle is the <br />sharps waste hauler for the facility I will also need a new training record for the <br />updated information. <br />Also, did you legally change your last name from Santos to Bambula-Santos?
