The incorrect and missing information included the following from the inspection reports: <br />-Reports were missing the name of UST operator/owner <br />-Secondary containment test date was not provided in the 9/23/22 and 2/8/23 reports. <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br />testing and maintenance for the UST system have been completed and shall check all required items on the report. <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br />all of the required information on the reports. <br />This is a repeat violation, Class II. <br /> 209 CCR 2715(c)(3), 2715(c)(4) Failure to maintain a copy of the "Facility Employee Training Certificate" on-site. <br />OBSERVATION: <br />-The Facility Employee Training Certificate dated 5/7/2022 did not list initial training and assumed responsibility <br />dates. <br />-The Facility Employee Training Certificate dated 5/5/2023 listed incorrect initial training and assumed responsibility <br />dates for Ryan Maldonado. <br />REGULATION GUIDANCE: All site employees must be trained prior to taking responsibility as a facility employee if <br />after October 1, 2018, or within 30 days of hire if prior to October 1, 2018.
