Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COGWBZTOK Site Location: 16460 ESCALON BELLOTA ROAD <br />Activity Summary <br />Activity Date Recorded by <br />03/12/2022 On-site to observe clean up of Cal OES Control #:22-1413 16460 Escalon Bellota ERNEST <br />Road. Appoximatly 1 gallon of indicated blue sticker non-PCB mineral oil spilled <br />from a pole mounted transformer due to a vehicle striking the utility pole. Jerome <br />Manuel, PSC was on site for clean up. PSC dug up the contaminated soil into a 55 <br />gallon black drum, wiped down contaminated fencing using rags which were <br />disposed of in the same black 55 gallon drum, and washed, scrubbed with a <br />brush, and wiped down contaminated concrete with rags. One black 55 gallon <br />drum containing solid hazardous waste, 5001b, was generated. Waste generate <br />will be taken to the PG&E: Stockton Service Center 4040 West Ln Stockton, CA <br />95204. <br />Pending analytical determination for waste generated on site and transfered to <br />PG&E: Stockton Service Center 4040 West Ln Stockton, CA 95204 <br />Received page at 11:00 am. Called Bryan Hoffman PG&E 530-440-5874. <br />Discussed spill, Bryan indicated the spill is 1 gallon of non pcb containing blue <br />sticker transformer mineral oil. No waterways impacted, but possibility of soil <br />impacted. Discussed that I would visit the site, and what contractor they was on <br />site for clean up. He indicated PSC was on site, and they would do the appropriate <br />job, and no further emergency services would be necessary. I discussed I would <br />be onsite to confirm clean up. <br />Traveled onsite and discussed clean up with Jerome Manuel 209 687 3204. He <br />discussed it would take a couple hours for clean up to start due to the utility pole <br />being repleaced. I left my car with phone number, and indicated if Jerome can, to <br />call me once clean up is started or to call an hour before clean up is completed. <br />Traveled back to Manteca. <br />Received call to show up and discussed showing up around 2:40-2:50pm. <br />Clean up initiated at approximately 3:10pm. Approximately 500 lbs of waste was <br />generated. Discussed with Brian Hoffman about sampling for pcbs. Brian H. <br />indicated because of the blue sticker generator knowlege will be used for <br />determining the type of waste disposal for the waste generated on site. <br />Generated a complaint number and notice to abate. Notice to abate was sent to <br />Alex Steele, Environmental Contact for PG&E: Stockton Service Center 4040 <br />West Ln Stockton, CA 95204, where the hazardous waste generated will be sent. <br />03/14/2022 Received phone call from Alex Steele Environmental Contact. Discussed notice to ERNEST <br />abate, and the spill clean up and sampling. <br />Called back and discussed the name of the person who notified the EHD of the <br />spill. <br />03/15/2022 Completed generation of prop 65, signed and added cc. Scanned prop 65 and ERNEST <br />forwarded to Elena Manzo. Started photo log. <br />02/08/2023 Found prop 65 for this complaint with a different complaint. No packet found from PRESTO <br />Fred Ernest's complaints. Printed 5104 and pinned together. Will review at a later <br />time. <br />02/13/2023 Reviewed narratives. Printed spill report from laserfische. Generated and printed <br /> <br />PRESTO <br />notice to abate from Fred Ernest's dailies. Found photos from folder of Fred's <br />photos.
