Food Program Service Request Inspection Report <br />Facility Name and Address: GUATELINDA MARKET LLC, 1201 E MAIN ST , STOCKTON <br />Environmental Health Department <br />NOTES <br />No major violations. <br />OKAY to issue permit once permit fee is paid and 5021 form is updated. <br />Program Element: 1616 = $257 <br />Note: Existing floor in the restroom was observed to be constructed of unapproved floor material but in good condition. When <br />the restroom’s floor materials are replaced in the future, ensure the new materials meet the California Retail Food Code <br />requirement: The floor surfaces in all food preparation or packaging areas, open food storage areas, utensil washing areas, <br />refuse storage areas, janitorial areas, and all restroom and employee change areas, shall be smooth and of durable <br />construction and nonabsorbent material which is easily cleaned. These floor surfaces shall be coved at the juncture at the <br />floor and wall with a three-eighths (3/8) inch minimum radius coving and shall extend up the wall at lea st four (4) inches. <br />Rubber or vinyl top set base is not permitted in these areas [CRFC §114268]. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
