VANG-LEE <br />09/09/2021 I contacted Adam Touchton of ETRS at 813-391-7322 at 6:30pm. I asked Adam VANG-LEE <br />what time the clean-up company would be on site. Adam stated Greymar were on <br />site now completing the clean-up. Adam provided me with contacts: Colton <br />Crosthwait 775-225-8123 and Jared Roberts 775-230-2172. I made contact with <br />Jared Roberts who informed me that they finished cleaning up and left the site <br />approximately 30 minutes ago. I asked Jared if any additional cleaning was <br />performed in the storm drain, Jared stated they did not clean the storm drain. He <br />stated that one 55 gallon drum of absorbent, booms, and pads was generated and <br />is being transported to their Reno yard. I requested for photos of the initial <br />manifest. Jared stated they were still on the road and would send me a photo <br />once they reached their destination. <br />I arrived on site at 7:10pm. I observed a few areas that still had staining. I took <br />photos and called Adam of ETRS. Adam referred me to Marcus Roy of ETRS. I <br />left a message for Marcus. I received a call back and expressed my concerns <br />regarding the clean-up being incomplete. Marcus informed me someone would <br />follow-up with me tomorrow regarding additional clean-up. <br />09/09/2021 Responded to a spill notification with Fred Ernest of approximately 20 gallons of NSO <br />diesel fuel due to a ruptured semi truck saddle tank on D'arcy Parkway in Lathrop. <br />Approximately 300 ft by 2 ft trail of diesel fuel was observed on the southern <br />shoulder of D'arcy Parkway. <br />Approximately 5 gallons of diesel was spilled in the storm drain. No soil was <br />impacted. City of Lathrop power washed and vacuumed the storm drain. <br />Greymar Environmental services was contracted by the responsible parties spill <br />broker for clean up. <br />09/09/2021 Received a call from Vicky concerning Penske roadside diesel spill, assisted with MANZO <br />spill clean up decision making process, received follow up update call at 7:31 pm, <br />discussed clean up progress. <br />5105 rpt Page 1 of 3
