OBSERVATION: Monitoring records for the last three years were not found on site. The following test results were missing: -2020, 2021, 2022 monitoring system certification -02/28/2022 overfill prevention inspection REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site. This is a repeat violation, Class II. 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. OBSERVATION: The following issues were observed on the monthly designated operator inspection reports: -The designated operator failed to check and note the correct test dates for the 07/2022 through 06/2023 designated operator inspection reports. -The 10/2/22 and 11/22/22 inspections were completed late -The 07/27/22 and 11/22/22 reports were missing the date the report was provided to the owner and were not signed by the owner/operator -The 10/2/22 report did not address the 09/02/22 alarms -The owner did not provide follow-up actions on the 12/22/22 report REGULATORY GUIDANCE:The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. During the inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Within 48 hours of the completion of the designated UST operator visual inspection,the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. The designated operator visual inspections shall be conducted within 30 days or less of the previous inspection. Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. This is a repeat violation, Class II.
