Please <br />provide the following additional information and address the following comments: <br />1) Page 3 of the Application for Underground Storage Tank Installation Permit: Provide the <br />worker's comp number and the fire district permit number. <br />2) Page 7- G (overfill), : Identify which overfill prevention method will be utilized at this facility, <br />Provide accurate manufacturer and model information, choose or indicate one of the overfill <br />method. If a mechanical flapper will be utilized, drop tube measurement calculations will need <br />to be provided prior to install. <br />3) Page 8 1 (monitoring) provide the dispensers and secondary piping monitoring sensor model <br />and manufacturer name. For tank monitoring model is it TLS 350 or TLS 350 plus, cut sheet <br />provided is TLS 350 plus. Page 8 says TLS 350. <br />4) Drawings: FS1-1 on General tank and line notes: says #2 install two UST tanks 1-15,000G 87, <br />and 1-20,000G split tank (91 10,000 , diesel 10,000) <br />Fuel key notes says 20k split tank UST for 87, permit application say 20K tank for 87. Correct <br />the drawings page. <br />Page FS2.0 says tank monitoring system TLS 350 plus, permit application says TLS 350, key <br />notes says TLS 350 <br />5) Complete training certifications were provided. Provide current contractor's training <br />certifications for installation and testing of all proposed UST equipment/components, including <br />but not limited to: tanks, sumps, sensors, monitoring panel (VR), leak detectors, spill <br />containment, and penetrations. <br />6) Provide manufacturer's cut sheets for all proposed UST system component, including but not <br />limited to: sensors, leak detectors, penetrations, and piping. <br />7) Submit a work plan for enhanced leak detection (ELD) testing, to be conducted by a properly <br />licensed individual. — already submitted but not signed. <br />8) Provide a drawing indicating the locations of all water wells, septic systems, and storm drains. <br />If you have any further questions, please contact me at (209) 616 3049. <br />Thank you, <br />1868 E.
