If you were trying to relate it to the bottom sentence regarding signs and symptoms of <br /> infection, I think you may have just forgotten to put a colon mark instead of a period at the end of the sentence. <br /> The following changes need to be made to the consent form: <br /> For number 3, please add on to it asking if they have other bleeding disorders but leave the hemophilia part in there. <br /> For number 14,change heart disease to"cardiac valve disease" <br /> For number 13,you need to ask if they have ever been prescribed antibiotics prior to surgery or dental procedures <br /> For number 16 regarding the description of the procedure, basically we need to know what the tattoo is(name, <br /> butterfly, etc.) and where it is on the body(left wrist, right shoulder etc.),so if you want you can erase the "tattoo or <br /> other" part and leave a line to write that information in there instead. <br /> You need a statement describing to the client what they should expect after the procedure. <br /> You need a statement that the inks are not FDA approved and health consequences are unknown. <br /> You need a sentence asking if they are allergic to antibiotics. <br /> If you have any questions let me know.
