<br />OBSERVATIONS: The kitchen back door has a gap to the outside along the side when closed as it was damaged by an
<br />attempted break in. Repair/replace door so that there are no gaps to keep facility vermin proof. Correct within 1 week.

<br />OBSERVATIONS: Observed debris and dust build up on ceiling, floor, and walls throughout faciltiy. Deep clean within 1
<br />week and maintain clean.
<br />Observed 3 missing ceiling vents in kitchen. Provide ceiling vent covers within 1 week.
<br />Observed damaged ceiling in back room above ice machine and storage shelf. Repair ceiling within 2 weeks.
<br />Observed damaged/missing coving in employee restroom and peeling coving in public restroom. Replace/repair with
<br />approved coving within 1 month.
<br />CALCODE DESCRIPTION: The walls / ceilings shall have durable, smooth, nonabsorbent, light-colored, and washable surfaces. All floor
<br />surfaces, other than the customer service areas, shall be approved, smooth, durable and made of nonabsorbent material that is easily
<br />cleanable. Approved base coving shall be provided in all areas, except customer service areas and where food is stored in original
<br />unopened containers. Food facilities shall be fully enclosed. All food facilities shall be kept clean and in good repair. (114143 (d),
