<br /> OBSERVATIONS:The entrance door to the kitchen was observed with peeling paint and was absorbent in spots. Paint
<br /> door with semi-gloss paint so that it is smooth, cleanable and non-absorbent within two weeks.
<br /> Grout is missing between tiles in the back of the kitchen near the two compartment sink. Re-grout area so that floor is
<br /> smooth, cleanable and non-absorbent within 30 days.
<br /> A base tiles was missing near the office and broken base tiles were observed under the kitchen hand sink. Repair/replace
<br /> base tiles with approved flooring so that base is smooth, cleanable and non-absorbent within 30 days.

<br /> surfaces, other than the customer service areas, shall be approved, smooth, durable and made of nonabsorbent material that is easily
