The law <br /> takes affect on July 1, 2012, and requires that Body Art shop owners, operators, and <br /> practitioners register with the local enforcement agency. In San Joaquin County, the <br /> enforcement agency is the Environmental Health Department (EHD). To implement the new <br /> Safe Body Art Program, EHD conducted a workshop on April 30, 2012, to provide an overview <br /> of the new law and the proposed program fees. The law contains new requirements for body art <br /> practitioners as well as facility design and construction standards. <br /> All practitioners must register with EHD by July 1, 2012, to be in compliance with the new law. If <br /> you have registered with EHD previously, you will need to re-register so that your record may be <br /> updated. If employed at more than one body art shop, you will only need to register once, but <br /> you will need to list all shops where you are working. There are no permit fees at this time. EHD <br /> will review and process the applications for registration, and if there are any unmet requirements <br /> or the application is incomplete, EHD staff will work with the applicant to complete the <br /> registration process. Based on the registration applications received, EHD will send the permit <br /> fee invoice to each owner, operator and practitioner in September 2012. EHD must receive <br /> payment prior to the issuance of the facility and/or practitioner permit, as the permit will be the <br /> proof of registration. The permit will be retroactive to July 1, 2012, and will be valid for one year. <br /> Enclosed are three registration forms: 1) Body Art Practitioner application, 2) Master File <br /> Record I (green), and 3) Master File Record II (pink). The shop owner is required to: <br /> 1. Complete the three forms (one package) for the facility and the three forms (one <br /> package) for each practitioner. Please submit the forms to EHD prior to July 1, 2012. <br /> 2. Please provide a copy of evidence of current Hepatitis B vaccination, including <br /> applicable boosters, unless the practitioner can demonstrate Hepatitis B immunity or <br /> has complied with current federal OSHA Hepatitis B vaccination declination <br /> requirements. <br /> 3. For new applicants, please provide six months verifiable experience along with your <br /> Body Art application. <br /> Thank you for your assistance.
