Miner Ave., Stockton, CA <br /> The Environmental Health Department(EHD) conducted a routine tattoo inspection at Black Rose Tattoo Parlor <br /> on 3/26/15. Several violations were observed and documented in the inspection report. Each violation and the <br /> required corrective action were discussed with the manager, Jose Zuniga. A copy of the inspection report with <br /> the documented re-inspection date was left with Jose. The EHD conducted the re-inspection on 4/27/15. <br /> Several violations that were observed and documented in the 3/26/15 inspection were still observed. A copy of <br /> the inspection report citing the continuing violations was left with you. According to the following code section <br /> of the California Health and Safety Code (H&SC): <br /> H&SC,Section II9320(d)states that: <br /> Whenever an enforcement officerfinds that a practitioner or body art facility is not in compliance with the <br /> requirements of this chapter, the enforcement officer shall issue a notice to comply or a notice of violation to the <br /> registrant or permitholder setting forth the acts or omissions with which the registrant or permitholder is charged, and <br /> informing him or her of a right to a hearing, if requested, to show cause why the registration or permit should not be <br /> suspended or revoked. <br /> Therefore, an Administrative Office Hearing has been scheduled on Monday May 18, 2015, at 10:00 a.m. at <br /> 1868 E. Hazelton Avenue, Stockton, CA 95205. An Administrative Office Hearing fee of $260.00 will be ' <br /> collected at the time of the hearing.
