Decontamination and sterilization areas shall be separated from procedure areas by a space of at least 5 feet or by a cleanable barrier and equipped with a sink with hot and cold running water, containerized liquid soap, and single use paper towels dispensed from a wall-mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(c)—The sterilization room was not supplied with containerized liquid soap. Ensure there is soap in the sterilization room. 11. The procedure area shall be equipped with a sink with hot and cold running water, containerized liquid soap, and single use paper towels dispensed from a wall-mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(b)— The soap dispenser, in the yellow room, was empty. Ensure the soap dispenser has soap, at all times. 18. The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are available for inspection. HSC 119303—A client record was missing a parent signature and some records did not have the lot number of the needle(s) used. Ensure all client records are signed by the parent(s) (for minors only) and lot numbers for the needles are written or the package is attached to the form. 31. A body art facility shall be separated from any residential areas used for sleeping, bathing, or meal preparation and shall not share a common entrance or toilet facility with residence. The toilet facility must have a sink with hot and cold water, containerized liquid soap, and single-use paper towels that are dispensed from a wall-mounted, touchless dispenser. HSC 119314—The restroom sink did not have hot water. Ensure there hot water is supplied in the restroom. A reinspection will be conducted to check for hot water. 32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to floor, from nail and hair activities and be separated from all business not related to body art. Approval of co-located body art facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each procedure area shall have lined waste containers. HSC 119314& 119317—The following was observed: • Restroom—Holes on the wall. • Blue Room— Holes near the sink. • Purple Room—Holes in wall and the area behind the table needs to be painted. • Green Room—There are holes in the wall under the TV. The floor and baseboard are in need of repair, next to the cabinet. Paint the purple room (as needed) and repair all holes and floor/ baseboard(s). Provide EHD photographic evidence of corrections. Unless otherwise noted, correct all violations and provide evidence of corrections to the EHD by October 1, 2022. Reinspection on/about: A reinspection fee of$156 per hour may be charged.
