The following is an itemized list of hazardous materials violations that have not been addressed for advance auto as of June 27, 2023. Open violations from April 06, 2022 inspection Violation #9 -Failed to annually review and certify that the HMBP is complete and accurate by the due date. OBSERVATION The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System (CERS) annually by the due date, January 15. - The 2022 business plan submittal was not submitted for year 2022, the last submittal was 11/24/2020. - The 2021 business plan submittal was submitted on time on 11/24/2020. - The 2020 business plan submittal was submitted late on 4/23/2020. REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. ❑ This violation was corrected ❑ Supporting documents included ❑ This violation will be corrected by (date): Describe actions taken or will be taken to correct violation: Violation #11 -Failed to provide or document initial and/or refresher training to appropriate personnel. OBSERVATIOI�t Annual training record documents were not observed on site during the inspection. Per Consolidated Emergency Response / Contingency Plan under section I. Employee Training "signed forms" are part of the employee training program. REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel receive initial and annual refresher training. All employees shall be trained in safety procedures in the event of a release or threatened release of a hazardous material, including, but not limited to, familiarity with the following plans and procedures: (A) Immediate notification contacts to the appropriate local emergency response personnel and to the unified program agency. (B) Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment (C) Evacuation plans and procedures, including immediate notice, for the business site. This training shall be documented electronically or by hard copy and shall be made available for a minimum of three years.
