5 t SANAAQUIN <br />- - COUNTY-- <br />cillS'epror %.3rtatoess grows here. <br />Environmental Health Department <br />Time In: 1:30 pm <br />Time Out: 2:10 pro <br />COMPLAINT INSPECTION REPORT <br />Name of Facility: CHINA EXPRESS (AT FOOD 4 LESS) Date: 05/05/2023 <br />Address: 8014 LOWER SACRAMENTO RD, STOCKTON 95210 <br />Owner/Operator QUINN, JOHN F Telephone (209) 956-3270 <br />Program Element: 1600- FOOD PROGRAM Complaint #: C00055810 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT STATES THERE ARE ROACHES IN THE ESTABLISHMENT AND THAT THE STAFF KNOW ABOUT IT. <br />COMPLAINANT STATES THAT WHILE SERVING A CUSTOMER A SERVER ALLEGEDLY KILLED A ROACH. <br />COMPLAINANT ALSO SAID SHE IS CONCERNED FOR THE CLEANLINESS OF THE ESTABLISHMENT, SHE STATES <br />SHE ALLEGEDLY SAW A SERVER EATING FOOD IN THE KITCHEN OVER CUSTOMER FOOD. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />#1 Demonstration of Knowledge <br />OBSERVATIONS: Some food handler cards were not available during the inspection. Provide food handler cards for <br />employees within 2 weeks and maintain records on site. <br />CALCODE DESCRIPTION: All food employees shall have adequate knowledge of and be trained in food safety as it relates to their <br />assigned duties. (113947) Food facilities that prepare, handle or serve non-prepackaged potentially hazardous food, shall have an <br />employee who has passed an approved food safety certification examination. (113947-113947.1) Any food handler hired after June 1, <br />2011 shall obtain a Food Handler Card within 30 days (113948). <br />#6 Handwashing Facilities Supplied and Accessible <br />OBSERVATIONS: The hand sink near the prep sink was blocked by a cutting board. Hand washing facilities must be <br />accessible at all times when operating. Corrected on site. <br />CALCODE DESCRIPTION: Handwashing soap and towels or drying device shall be provided in dispensers dispensers shall be <br />maintained in good repair. Clean within 1 week and maintain clean. <br />CALCODE DESCRIPTION: All nonfood contact surfaces of utensils and equipment shall be clean.
