Approximately 50 gallons of mineral oil released onto asphalt and flowed <br />into a storm drain. The release was caused by a vehicle accident. Alex said he <br />notified the City of Stockton (Stockton). Viewed the area on parcel viewer and <br />found it was within city limits. Called Brian Toledo and he said that several people <br />from Stockton were on their way to the site. <br />Left for the site with Paul Nso, EHS trainee. Arrived at 2:30 PM. <br />Mineral oil was observed leaking out of the transformer and oily staining was <br />observed on the parking lot. PG&E put absorbent on the asphalt to soak up the <br />mineral oil. The mineral oil flowed into a sump that was connected to a storm <br />drain. Brian Uribe arrived on site and verified that the oil did not leave the sump. <br />The connecting storm drain was blocked to prevent any oil from entering the <br />system. Dave Larsen said that PG&E hired PSC to clean the spill. PSC will be <br />using a vacuum truck to clean the sump. <br />The business at the location of the spill is Guarantee Auto Repair. Made contact <br />with the owner of the business and property. The owner's name is Faizal Awadan <br />and his phone number is 209-466-7664. Called the owner and discussed the <br />cleanup. He said he was going to head to the site to view the cleanup. I told <br />Faizel to call me if he had any concerns once the cleanup was completed. <br />Dave Larsen and Brian Toledo confirmed the EHD would be notified of completion <br />of cleanup. <br />3:18 PM Departed site. Got back to the office and created complaint, notice to <br />abate, Prop 65, and photo log. Emailed information to inspector on nighttime ER. <br />The notice to abate was emailed to Alex Steele. <br />05/18/2021 Provided John Alaniz, Sr REHS, with a status update on the clean up. FLORIDO <br />05/18/2021 Received a call from Alex Steele informing me that the spill cleanup was almost ALANIZ <br />completed. <br />Arrived on site at approximately 10:30 PM. Met with Alex and Brian Toledo. The <br />sump and connecting storm drain line was cleaned. 100 gallons of waste water <br />was generated and is going to be disposed of as non-hazardous waste. I asked <br />Alex to send me the disposal records and PCB results. <br />05/20/2021 -Received PCB results from Alex Steele on 5/19/2021. The oil sample came back ALANIZ <br />as non-detect. <br />-Completed photo log and narrative from 5/18/2021 site visit. <br />05/10/2023 Reviewed complaint. Sent an email to Alex Steele, PG&E, asking for the disposal ALANIZ <br />records. <br />05/15/2023 Received the disposal record from Alex Steele, PG&E. Reviewed the document. ALANIZ <br />Created the site map. Gathered the complaint documents and abated it. A note <br />was made in PRO513829 for the waste that was taken to 4040 West Lane. <br />5105.rpt Page 2 of 2
