A release noticed by staff at approx. 2:45am <br />during night shift The site was evacuated. Discussed possibly not making it out to the site due to scheduling issues. <br />06/29/2022 Reviewing notes and narratives from fridays investigation. ERNEST <br />Reviewed incident investigation report and leak calculations for ammonia. <br />06/29/2022 RECEIVED SPILL REPORT CALOES#22-3588 AT APPROXIMATELY 9:15AM. ERNEST <br />CALLED NICHOLAS CABRENA, EH&S MANAGER WHO INDICATED THE <br />AMMONIA RELEASED WAS MITIGATED. THE DAMAGED VALVE WAS <br />ISOLATED AND REPAIRS WERE TO BE CONDUCTED ON 6/24/2022. <br />RECEIVED INCIDENT REPORT AND LEAK CALCULATION SHEET. INCIDENT <br />INVESTIGATION REPORT INDICATED 3 LBS OF AMMONIA WAS RELEASED <br />FROM A VALVE ON WINE TANK 369. NO EMPLOYEES WERE EXPOSED, <br />INJURED, OR KILLED.
