Complaint Investigation Summary <br /> Report #: 5105 <br />COMPLAINT ID: C00055808 Site Location: 2401 MACARTHUR DR, TRACY, CA 95376 <br />Activity Summary <br />Activity Date Recorded by <br />05/01/2023 Ben Schlegelmilch, Plant Engineering Manager sent me an email on 4/28/23 at SAEED <br />2:32 pm stating," Last night at around 7 pm, we had a small ammonia leak of 7 <br />lbs. in one of our freezer evaporators. Once the maintenance team was notified of <br />a smell, they immediately investigated, found the source, and isolated the <br />evaporator." <br />I was out of the office on Friday. When I returned to the office today, I replied to <br />the email stating, ". Please follow the facility emergency response plan/emergency <br />action plan for reporting releases by calling our main number (209)468-3420 and <br />notifying Cal OES". <br />At 10:46am I called Ben to gather more information about the release. <br />The release occurred at the storage freezer in Evaporator #8. The evaporator coil <br />leaked ammonia and ammonia odor was reported by facility personnel. The leak <br />was isolated by the facility maintenance personnel. Evaporator #8 is currently out <br />of service. Incident investigation is already started. <br />While I was discussing this ammonia releases with Ben, he mentioned that they <br />had another ammonia release yesterday. The release occurred in the ice cooler <br />due a crack inside the pressure gauge. The root cause is still under investigation. <br />The amount of ammonia released to the atmosphere calculated to be 11 lbs. <br />Incident investigation is already started. <br />No exposure or evacuation due to the incident. The release did not go offsite. No <br />ammonia detector within the area. <br />Explained to Ben that they have to follow the Cal OES spill reporting requirements <br />and their emergency response plan for reporting release. <br />05/01/2023 Discuss complaint details with Lead Senior (Haza), Kristina and facility operatior. NSO <br />05/02/2023 Generated a generic complaint report and emailed it to Ben Schledgelmilch: <br />bschlegelmilch@leprinofoods.com and Doug <br />Chitwood:cchitwood@leprinofoods.com <br />The report included a corrective action to submit incidents investigation report <br />within 30 days. <br />Complaint will be abated to the facility CalARP PR#. <br />Confirmed receiving the report by calling Doug. Discussed the incident and initial <br />corrective actions with Doug. Notes included with CalARP PR# for follow-up. <br />SAEED <br />5105 rpt <br />Page 2 of 3
