A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of practitioner and <br />client, date of procedure, instruments used, and written evidence from the manufacture that the instruments have undergone <br />sterilization. The record of purchase, evidence of sterilization, type and number of instruments used shall be maintained for a <br />minimum of 90 days after use. HSC 119315(f) — An invoice was not available for the Solong needles with lot #CP002086. <br />An invoice and sterilization certificate was not available for an unknown brand of needle cartridges with the lot <br />#2021/04. Provide EHD with invoices for both needles and a sterilization certificate for the needles with the lot #2021/04. <br />19. At the completion of the procedure, the practitioner shall provide post procedure instructions that include all elements listed <br />in Form B. HSC 119308(b) — The post procedure instructions did not include all the elements listed in Form B. Include <br />the following on the post procedure instructions and provide a copy to the EHD: <br />- Signs and symptoms of infection. <br />- Indications when to seek medical care. <br />32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br />The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br />floor, from nail and hair activities and be separated from all business not related to body art. Approval of co -located body art <br />facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br />procedure area shall have lined waste containers. HSC 119314 & 119317 — The garbage can in the procedure room was not <br />lined. Line the garbage can and provide evidence of correction to the EHD. <br />Correct all violations and provide evidence of correction to the EHD <br />by April 4, 2022. <br />Notes: <br />1. Inspection report emailed to Lorlie Ty, owner, at IorraintvCcDvahoo.com. <br />2. No more than one student will be trained at a time, per facility owner. <br />3. Students are required to be trained on the Infection Prevention and Control Plan. The record of training shall remain at the site <br />for 3 years. <br />4. Sharps last mailed back in January 2022 to GRP. <br />5. If sterilization certificate is not available for the unknown brand (#2021/04) of needle cartridges then the practitioner will not <br />use the needles and notify EHD. <br />Reinspection on/about: <br />A reinspection fee of $152 per hour may be charged.
