Please view the 3 <br />attachments. Wall finish is ultra durable and ultra wipeable paint in a satin sheen. The floor is an ultra durable <br />waterproof vinyl material made to look like hardwood. baseboards are caulked and sealed to the wall. I am <br />paying the fee after i hit send on this email. A completed Service Request Form <br />2. A layout of the facility, showing the location of the restroom, sink and procedure table/ chair. A hand drawn <br />layout is fine. <br />3. A description of the wall finish and type of flooring used in the facility. <br />4. A completed Infection Prevention and Control Plan <br />5. There is a fee of $152 for the consultation. The fee can be paid online at <br />https://www.sigov.ory,/department/envhealth/online-payment or in person at the EHD building.
