Called the <br />complainant and they stated they found a piece of glass the size of a quarter in <br />their omelette on 5/27/2023. The complainant tried to get the shard back but the <br />restaurant kept the glass and receipt and put it into a locked box in the manager's <br />room. Per the complainant, the glass was clear and they had not sustained any <br />injuries. <br />Discussed complaint with Jeff Carruesco, Program Manager. Did not go to site. <br />TIME SPENT: 11:48 AM-12:18 PM <br />Created the complaint, finished the narrative, and emailed it to Jeff for further <br />follow-up. <br />05/30/2023 Arrived at restaurant located at 18700 N HVVY 88 located in Lockeford. Discussed MURO <br />the alleged complaint with the manager. The manager connected me to the owner <br />via phone call. Owner was aware of the complaint and reported that the piece of <br />glass was actually a piece of plastic that came off of the onion packaging. Owner <br />stated employees will take extra measures on food safety and granted inspection <br />of restaurant. Verified employee's food handler certificates are current. Per chef, <br />omelets are prepared when an order comes in and are prepped on the prep table <br />adjacent to the cook line. Eggs are stored inside the walk-in cooler, currently at 37 <br />F until ready for use. Did not observe any glass or plastics around egg prep or <br />storage areas. Verified orange juice to be stored in the walk-in cooler, currently at <br />37 F and within best by dates. The orange juice dispenser was observed with <br />orange juice spillage.
